Flex Physical Therapy will maintain the personal privacy of patients at all times, insure compliance with privacy standards of HIPAA, and exchange health information in order to make efficient administrative health care transactions while protecting the privacy and security of those transactions.
We may use your medical information or disclose it to others for treatment, obtaining payment for treatment, conducting internal administrative activities related to or supporting your treatment. Your personal health information is to be disclosed with another covered entity at the minimum necessary to administer business. Personal health information is not to be shared with non covered entities unless authorized by the individual or required by law.
Flex Physical Therapy may use or disclose your personal health information without prior authorization permitted by certain state or federal laws, regulations or legal circumstances for public health purposes or emergencies.
You have rights regarding your medical information as a patient at Flex Physical Therapy. You may review, obtain a copy or correct any inaccurate or incomplete information in your record at any time.
You have a right to request a list of instances where we have disclosed your personal health information for reasons other than treatment, payment or other related administrative purposes.
If as a patient, you believe your privacy rights have been violated or disagree with any decisions that we have made regarding access or disclosure of your personal health information, you may file a complaint in writing or contact the Privacy Officer and the complaint will be met with full respectful attention without retaliation.
